How to enable debug output:

1. go to Edit->Preferences->Advanced->General->Config Editor…
   
2. change the setting of calendar.debug.log to true
   
3. run Thunderbird from a console and capture what is printed

Be sure also to report any exceptions that pop up in Tools->Error Console
